If your student is experiencing a medical or psychiatric emergency, please call 911.

You can additionally complete the following form if you are concerned about someone’s health or safety.
Grade Concern
Student is receiving a D or F and is in jeopardy of failing the course, or is considering withdrawing from the course.
Missing/Late Assignments
Student consistently has missing/late assignments that are affecting their grade.
Attendance Concern
Student is consistently absent and it is affecting their grade.
Participation/ Preparedness Concern
Student is not participating in class and it is affecting their grade or student struggles with having materials needed for the class.
Well-being Concern
Student is displaying behavior that raises concerns about their well-being (please submit additional comments).
Recognition
Student has recently engaged in something positive that deserves recognition (to be submitted alongside a topic of concern).
